A mechanic was on top of the HHP in Bay 1 drilling out broken bolts. Another operator was running the overhead crane using a handheld remote, moving it from the south side of the HHP to the north side to pick up a fluid end. As the crane moved over the top of the HHP, it struck the mechanic, knocking him off the HHP onto the cement floor of the shop 13 feet below. The injured employee was hospitalized for broken ribs, a sprained wrist and stitches to the face.

The kelly cable drive was not retracting properly on the machine's drum, so the employee tried to disconnect the cable to re-spool the drum. He backed out the threaded pin from the kelly drive clevis that was holding the cable drive end. As he tried to push the pin through, his right middle finger was pinched in the clevis and cable. The finger was amputated at the first joint.

When the employee flipped the third beam it landed at an angle and fell off the saw horses and struck and fractured the employee's right leg and left ankle. The employee was hospitalized for surgery on his left ankle.

An employee was standing at the edge of an approximately 48-inch diameter hole that was approximately 18 feet deep. The employee was using a winch cable off the back of a drilling rig with a hook on it, attempting to retrieve a shovel that had fallen into the hole, when he lost his footing and fell approximately 18 feet to the bottom of the hole. He broke his right leg in two places with possible additional injuries to the left leg.

As they were removing a cut-out portion of the well casing from the drill floor, the casing was bumped and shifted, causing it to fall enough to take slack out of the cable. The injured employee had placed his hand on the cable to remove it from the hook and his finger was caught between the hook and the cable. As a result, the distal phalange of his left index finger was amputated.

An electrician was relocating a live, 480-volt wire in an electrical panel. Carpenters from another company were drilling a hole on the floor with a track underneath the panel. An arc flash occurred in the electrical panel and hit the electrician and two carpenters. The electrician sustained burns to his face and stomach. One carpenter sustained burns to his back. The other carpenter had buzzing in his ear due to the loud sound.

At 5:15 AM, the crew had started drilling horizontal holes in an island slab. At 5:45 AM, an employee noticed a hole had been drilled 1/2 inch off from the correct location. To correct the issue, an employee grabbed a drill, thinking that it had stopped running, but the drill was still on. The employee's glove was caught in the drill bit resulting in an amputation of the top of the employee's middle finger.

The injured employee was holding a cart carrying a 10' section of drilling pipe. A crew attached a snatch block to the section of pipe to lift the piece and connect it to the rig to continue digging deeper. As the crew began to lift the section of pipe off the cart, dirt had accumulated on the chain causing the pipe to fall around 6 inches and crush the injured employee's right hand between the cart and the pipe. The employee suffered fractures and lacerations to his fingers.

The injured employee was standing next to the drilling piping, holding material over the pipe joint to channel residual mud downward, ready to swing the disconnected piping out of the way. Oil-based drilling mud came out under pressure and three employees got mud in their eyes when their hard hats and safety glasses were blown off. One employee was hospitalized for an eye injury. The eye was burning, swollen and the employee was unable to see.

The injured employee was working on the catwalk pedestal of an oil and gas drilling rig. While lifting a single joint of drill pipe, it fell out of the elevator and came down on the injured employee's left hand which was on the handrail. The employee sustained lacerations, fractures and nerve damage to the left hand.
