An employee was installing gutters on a newly constructed residential dwelling. The employee was climbing a portable extension ladder to install a downspout over the garage and fell approximately 10 feet head-first onto the concrete driveway. The employee sustained a skull fracture, swelling of the brain, eight broken ribs and a collapsed lung and was hospitalized.

An employee was installing a downspout from a ladder. The ladder was leaning on the side wall of a two-story house. The employee dropped the downspout and tried to catch it with his foot. The employee lost balance and tried to get his foot on the ladder, but missed the rung. The employee fell 12 - 15 feet from the ladder to the mulch bed resulting in fractures to his left arm, the first two left ribs and left scapula. The employee was hospitalized.

An employee was helping a press break operator form a piece of sheet metal for a gutter downspout. The operator and the employee placed the sheet metal into the break press and aligned the markings that were on the sheet metal under the press bar for the proper bends. After the sheet metal was aligned in the press, the operator activated it at the control panel. The employee's right hand was in the press, and his two middle fingers were amputated at the first joint.
