As he pulled the cable to close the door, the cable wrapped around the yellow safety pole and was pulled taut against the pole. The employee's left middle finger got pinched between the cable and the pole and the fingertip was severely lacerated. The employee required hospitalization and surgery.

As he attempted to evade the cart, he lost balance and fell over the edge and into the dumpster, landing on the bottom about 4 or 5 feet below and suffering a broken sacrum.
