The sheaves of the crane were frozen due to ice and snow conditions, which caused a backlash with the auxiliary cable of the crane. The operator attempted to straighten out the backlash on the superstructure near the drum. The operator sustained an open fracture of the humerus and a dislocation of the radius.

An employee was preparing a trailer to load a roller that was on a slope. A coworker was in control of the roller and left it unattended. The coworker set the emergency brake, but the emergency brake was not working properly. The roller then began to roll slowly and struck the employee, who suffered a leg/hip dislocation.

An employee experienced an amputation of the left pinky finger while guiding a rod out of a well. The rod was sticky and grabbed the employee's gloved left hand and spun it counterclockwise resulting in the amputation of the pinky finger and dislocation of the ring and middle fingers.

On or about 14 April 2015, at approximately 1612 hours, a Troy Vines, Inc. employee was replacing the tire on a Redimix truck on Tradewind and Duval Street. He lowered the vehicle to reposition the jack and was caught between the road surface and the truck's structure. The compression caused a hip dislocation and a fracture in his hip area. He was hospitalized.

An employee, in the process of dismantling a scaffold, climbed the scaffold ladder to approximately 25 feet and stepped off onto a pipe header. Once on the pipe header, the employee slipped off and fell 25 feet to the ground below where he suffered a right arm compound fracture, a right elbow dislocation, and a fractured pelvis. A yo-yo tie off was available that the employee was not using. The incident occurred at the 100 rack at the enterprise construction site.
