At 10:00 a.m. on January 6, 2021, an employee was operating a 55-gallon drum crusher when the ram of the crusher pushed the ½-inch metal door outward, breaking the door welds and striking the employee in the head. The employee suffered head trauma and was hospitalized.

An employee was exiting a rock crusher and getting onto a 6-foot step ladder. He was holding onto the handrail while getting his feet onto the ladder. His feet were on the second rung from the top when the ladder kicked out from underneath him. He held onto the handrail momentarily and then fell 4 to 5 feet to the ground (gravel pad), breaking his left wrist and suffering a small compression fracture in his lower back. He was hospitalized.

Two employees had shut down a can crusher because the auger was making a funny noise. The injured employee had climbed up to inspect the upper bearing and had set his right foot over to the baler to steady himself. The baler engaged and started down, and the bale ejector chain hook came down on his boot behind the steel toe. His right big toe was amputated.

Employee received a crushing injury to the right-hand thumb when the collapsible railing on a crusher plant's catwalk vibrated loose and pinned his thumb between the railing and catwalk. The thumb was partially severed and the bone broke. His thumb was later amputated at Rochester Mayo Clinic because blood flow to the thumb could not be re-established.
