An employee was cutting bluestone with a wire saw. As the employee used a crowbar to get the wire out of the block, the piece of stone that was being cut broke in the middle, fell at an angle, and rotated down. It struck the employee's right foot, causing amputations to the second and third toes.

An employee was using a extruder for processing soy when an extruder screw became clogged with soy. The employee used a crowbar approximately 18 inches long to unclog the extruder when the extruder grabbed and pulled the crowbar, resulting in severe injury to the employee's right hand, including amputation of their middle, ring, and pinky fingers.

An employee was removing an excavator bucket pin with a crowbar. The crowbar slipped and struck the employee, who fell and suffered a cut on the right side of his face, a broken nose, a possible broken right upper cheek, sore neck muscles, and a concussion. The employee was hospitalized.

An employee and a second employee were cutting a log with a chain saw next to a storm water ditch. The employee walked to the downhill side of the log to position a 6-foot crowbar to assist with the log cutting. When he repositioned the crowbar, the log began to roll towards him and he fell into the ditch (approximately a 2-foot dry ditch). While he was in the ditch, another log rolled down. The second log crushed him and pinned him to the ground. He suffered a fractured pelvis and punctured bow

An employee was using a crowbar to lift the back left side of a section of electrical switchgear cabinet to install a shim. The crowbar rolled, and the weight of the gear came down onto the tip of the flat piece of metal he was pushing the shim with. His left index finger was pinched between the shim metal and the concrete pad amputating the fingertip.

An employee was using a crane to move loads of pipe from the shop floor onto the shelving system in the warehouse. He noticed that one of the pipes in the load was stuck on the lip of the warehouse shelf. He climbed a step ladder and attempted to unhinge the pipe using a crowbar. When he used the crowbar, the load began to shift on the shelf and he jumped from the ladder to avoid it, landing on his left leg on the concrete shop floor. He fractured his left knee and tibia and was hospitalized.

On June 8, 2017, an employee was holding one side of a manhole cover while a coworker lifted the manhole cover with a crowbar on the opposite side. The manhole cover slipped off of the crowbar hook and fell onto the employee's hand, pinching off the finger pads of his right middle and ring fingers between the manhole lid and lip. His ring fingertip had to be surgically amputated.

An employee was underneath the trailer tongue frame of a portable abrasive blasting unit, attempting to straighten a bent pin in the jack stand with a crowbar. As he pried on the pin with the crow bar, the jack fell, pinning him beneath the trailer tongue frame. He punctured his lung and broke five ribs, his sternum, and his left shoulder blade, requiring hospitalization.

On 7/1/15 an employee was working at the Firestone Plant in the scale house, when it was discovered that a gate on the asphalt silo had a broken bolt and was not functioning properly. At ~ 12:40 AM while attempting to replace the bolt, his crowbar slipped and struck him in the face. He was admitted for in-patient hospitalization for a fractured bone above the eye (no eye loss/damage) and a fractured cheek bone.
