The employee injured their collar bone and scapula and sustained a cut on the head and a puncture wound on the back.

Employees were attempting to move a crate of six granite slabs (weighing approximately 800 pounds each) from the back of a trailer using a chain. One end of the chain was tied to the mast of a forklift and the other wrapped around the bottom of the crate. Before they pulled the crate from the trailer, a strap that was holding the slabs broke, allowing the slabs to fall onto the injured employee, who broke both legs.

An employee was helping a coworker rearrange a crate (weighing 706 pounds) with an electric pallet jack. He was standing at the end of the forks and leaned the wooden crate to get it off the forks. His coworker then lowered the forks, which bumped the ground, and the crate tipped. He tried to stop the crate from falling, but it fell on his right leg, fracturing it between the knee and foot, and requiring hospitalization.

A truck driver and an employee were holding a toolbox crate on a truck lift gate. The toolbox was too heavy for the lift gate. A second employee was taking things out of the toolbox to lighten the load. The truck driver then let go of the toolbox. It started to roll off the lift gate and fell on the first employee's feet, fracturing his left foot and requiring overnight hospitalization.
