A mechanic was on top of the HHP in Bay 1 drilling out broken bolts. Another operator was running the overhead crane using a handheld remote, moving it from the south side of the HHP to the north side to pick up a fluid end. As the crane moved over the top of the HHP, it struck the mechanic, knocking him off the HHP onto the cement floor of the shop 13 feet below. The injured employee was hospitalized for broken ribs, a sprained wrist and stitches to the face.

An employee was lowering machinery into a vat of caustic chemical using an electric hoist and crane. An I-bolt (screwed into the heavy machinery and also being held by the hoist) broke, and the machinery dropped into the vat.
