An employee was helping to train two new employees on how to change hammers in a hammer mill. The employees conducting the training were removing the hammers, which requires pushing a steel rod through the hammers and collars to push out another rod that holds the components in place. Employees were loading 6.5-inch drill collars onto a 40-foot flatbed trailer for transport. The injured employee was attempting to install a band around a collar to secure it when the drill collar rolled off the trailer and struck the employee's right foot. As a result, five of the employee's toes were amputated.
