An employee was about to start winding the core of a hose on a reel, which requires attaching the end of the core to an air inlet on the side of the reel. The reel becomes automated once the hose core is attached and begins to spin. When she went to attach the core to the air inlet, the reel began to automatically spin. Her hand was caught between the spinning reel and a clamp that holds the pin in, resulting in a fracture to the second metacarpal bone of the left hand.

The machine had not completed the clamping process and her right index finger was caught between the upper electrode and the ring. The fingertip was amputated

While turning the tensioning screw, the employee’s right hand slipped and his right ring finger contacted the square tube blade resulting in a fingertip amputation without bone loss.
