He was hospitalized.

The hand was torn open and the top part of the index finger was medically amputated.

The employee fell, and their right little finger became lodged between the chute and a metal shield used to prevent product from falling off the conveyor. The fingertip was amputated at the last joint.

The employee was hospitalized for a concussion and fractures to the left leg, pelvis, ribs and face.

His left hand slipped and entered the motor where the impeller blades struck and amputated the fingertips of his index, middle, and ring fingers.

The metal grabber moved away from the employee, and the employee lost balance and fell to the floor, landing on the right side. The employee sustained a contusion to the right elbow.

He suffered a fractured pelvis and broken ribs, as well as a laceration above the eye.

The employee sustained a leg hematoma that required surgery.

On July 29, 2019,at 10:30 a.m., an employee was preparing to add sugar product to a metering chute. He reached up into the metering chute to verify the equipment was off and his right middle finger was struck by a pneumatic slide gate. His finger was fractured and lacerated, resulting in a surgical fingertip amputation.

As he was descending from the ladder, his feet became tangled on the fourth rung and he fell backwards about 3 feet to the gravel floor. He injured his left ankle and lacerated the back of his head, requiring hospitalization.

An employee was troubleshooting and performing maintenance to the limit switch on a casting machine trough. The trough became energized causing the trough (and the attached chute) to turn over and strike the employee in the head/neck. The employee was then pulled under the trough. The employee suffered broken ribs, a broken chest plate and a punctured lung.
