One employee opened an access panel to the drum of the chipper to unclog it while the second employee went to the other side of the chipper. The first employee then shut the access panel. The motor was off, but the drum was still spinning. A piece of clogged brush kicked the panel open and struck the second employee in the forehead, resulting in multiple skull fractures.

On 5/7/2019, an employee was working on the ground level collecting falling brush from a tree and disposing of it in a wood chipper. A second employee was performing tree pruning and utility line clearance operations in a utility truck bucket approximately 45 feet high. The second employee began to cut a tree lead, approximately 15 feet in length, with a chainsaw. The tree lead then fell and struck the first employee's face, causing nose and facial lacerations, fractures, and contusions.

This caused the bottom of the lid to rotate and roll over the bump stop. The lid then rolled over towards the employee, striking the employee in the left arm and the front of his hard hat. The employee sustained a broken left humerus.

Workers were performing tree trimming activities at a residential jobsite. While moving traffic cones that were stored on a wood chipper, an employee's right hand contacted the chipper's fan assembly, amputating his fingertips.

An employee was picking up cut brush and placing it into the chipper during power line right of way clearance. The branch brushed his abdomen and lodged a thorn into his abdomen. The event occurred on Sept. 22, 2015. Several days and two doctor visits later, the employee was admitted to the hospital on Sept. 29, 2015 with a severe infection. The thorn was found during exploratory surgery.

An employee was repairing the chipper with the guard off and the machine locked out and tagged out. The guard was replaced and the machine was started. He heard a noise and again powered down the machine and took the guard off. He was leaning up and into the chipper to feel for vibration and his fingers got caught in the blades before the wheel had come to a complete stop amputating the fat pad and the fourth left fingertip.

A flash dropped down into the core pool area; the employee put the table in manual mode so he could manually remove it. The other employee at the table misunderstood what the employee was doing and actuated the part of the turn table, which caused the core to move and amputate the injured employee's right middle finger at the first knuckle.

The employee was tightening a belt on the wood chipper. After the repair, the employee turned the machine off. While the machine was coasting to a stop the employee got his left hand caught between the belt and pulley of the wood chipper, resulting in the amputation of two fingers.
