The finger was pinched and crushed between the tubing and a piece of channel on the workbench, causing an amputation to the fingertip above the nailbed.

At about 6:00 p.m. on July 23, 2021, an employee was using a forklift to empty a pellet hopper into a roll-off box. The hopper bounced back and the employee's left index finger was pinched between the forklift fork and the hopper's channel iron base. The employee suffered an amputation to the finger.

An employee was standing on a 14-inch, 60-foot-long, 73-pound-per-foot H-pile lying on dunnage. A rough-terrain forklift was being used to move a 6-inch, 12-foot-long channel on its forks. The end of the channel clipped the end of the H-pile, causing it to roll off the dunnage and strike the employee's lower leg. The employee suffered fractures to the lower leg and ankle, with no open wounds.

After drilling holes, he manually flipped them over. When the employee flipped the third beam it landed at an angle and fell off the saw horses and struck and fractured the employee's right leg and left ankle. The employee was hospitalized for surgery on his left ankle.

While working in the Rigger Shop, an employee was using channel lock pliers to disconnect two pieces of pipe that were bolted together. The pipe was raised by a crane. When the top bolt was removed, a portion of the pipe was pulled upward, and the other portion fell to floor. The employee's finger was pinched between lower handle of pliers and the portion of pipe supported by the crane. The employee suffered an amputation to his left ring finger.

When he moved the level to dump the hopper, the weight caused the hopper edge to drop and crush his left pinky fingertip, which had to be surgically amputated.

An employee was changing grease fittings and lines on a stand in the plant. The employee was picking up the hoses as he was walking away (there is a channel that the profile gauge moves back and forth over). The profile gauge had been moved away at the time, uncovering the channel. The employee stepped in the open channel, which is 26" deep, and broke his left leg.

An employee was putting wire on a U-shaped, steel channel frame, balanced on forklift tines, in order to attach the frame to an overhead crane. The steel frame began to fall backwards, and the employee tried to maintain its balance causing the employee's right pinky finger to get caught between the forklift frame and the fork tines. The employee's right pinky finger was removed from above the second knuckle.

An employee was cleaning a wheel feed guide. As he exited the area, his hand got caught in the feed channel wheels, crushing and amputating his left thumb.

The employee was working in maintenance utility area when he stepped through a door and fell a little over three feet to the bottom of an air channel. He fractured his right fibula and tibia.

An employee was troubleshooting the vacuum channel of a machine when his hot gloves got pulled into the blocks. Half of his right ring finger was amputated. The guard was not in place at the time.

An employee was piloting a boat to bring crew members back to shore. The boat navigation systems indicated that they were farther out in the channel than they actually were. The boat hit a tree and threw two employees onto dry land. One of the two employees was hospitalized.

An employee was presetting a roller channel machine. While presetting the chuck, a jam occurred. The employee tried to clear the jam. The pusher activated and pinched the tip of the employee's right index finger. The employee went to the emergency room, which documented the injury as an amputation.
