An employee was reroofing a flat roof of a single story dwelling. The employee went to get a chalk line from the ground. He then climbed back up the ladder and knelt down on the edge of the roof to snap the chalk line. The roof gave way and he fell 8 feet 4 inches onto a pile of landscaping posts and sustained fractured ribs. The employee was wearing fall protection, but it was not connected at the time of the fall.

An employee was striping lines in a soccer field. He placed a metal stake in the ground, using string as chalk. The string did not reach as far as needed and when the employee tugged on the string, the stake came out of the ground like a slingshot, striking his left bicep. The stake traveled through his arm and punctured his lung.

On or about July 28, 2016, at 7:00 a.m., an employee was on the second landing of a scaffold on the side of a building, preparing a chalk line to put a V-groove in a column. When he stood up, he became dizzy and fainted. He fell forward toward the building and onto a brick ledge, fracturing several vertebrae, his left scapula, and several left- and right-side ribs. He was hospitalized.
