An employee was lowering a 120-pound helical screw into a centrifuge using a chain fall suspended from an I-beam. When the screw fell quickly into place, the employee's right ring finger was caught between it and the shaft. The fingertip was amputated.

The injured employee was using an oxy-acetylene torch to remove one of the legs from the outside of a centrifuge (used process equipment).
