An employee was about to start winding the core of a hose on a reel, which requires attaching the end of the core to an air inlet on the side of the reel. The reel becomes automated once the hose core is attached and begins to spin. When she went to attach the core to the air inlet, the reel began to automatically spin. Her hand was caught between the spinning reel and a clamp that holds the pin in, resulting in a fracture to the second metacarpal bone of the left hand.

The injured employee was holding a trailer stake and when the forklift operator lowered the load onto the truck, the employee's hand with the stake drifted under the trailer. The trailer came down and crushed his hand between it and the stake. He suffered a fracture to his left ring finger metacarpal and was hospitalized.

The door could only be raised or lowered using the manual controls. While the employee was clearing the scrap paper from the sensor, his coworker activated the controls to lower the door. The door closed on his left hand, causing multiple metacarpal fractures and lacerations.
