An employee was applying aluminum caps to the corners of cardboard material using a riveting machine. The employee went to grab a cap that had popped off a corner when the foot pedal was activated and the employee's left index finger was crushed by the riveting machine, resulting in a fingertip amputation. A guard was in place at the time.

At about 1:30 p.m. on May 27, 2021, an employee was carrying a 4-by-6 plywood panel down stairs from a second floor to a mezzanine. The employee fell at the last or second-to-last step and landed on the floor. Both of the employee's knees were injured, with quadriceps tendons torn from the kneecaps. The employee was hospitalized and underwent surgery.

He went to step into the job site trailer when his left foot became stuck on the threshold, causing him to lose balance and hit both knees against the wall of the trailer. An employee was operating a loader with a grapple attachment to unload tree logs from a truck when he noticed the temperature indicator light flashing. The employee turned off the loader and waited a few minutes to let the engine cool off. Next, the employee started to remove one of the radiator caps on the loader. As he started removing the cap, hot water and steam released from that area, striking the employee. He sustained third degree burns to his left wrist, left arm, and left side torso.

A maintenance employee was working on the roof. He was walking down the fixed stairs from the cooling tower when he missed a step and both his knees gave out. He caught himself on the rail and did not fall. He suffered from torn tendons behind both kneecaps, requiring hospitalization and surgery.

On February 4, 2019, an employee was removing a sheet of aluminum from a pile of aluminum (~ 4 feet x 10 feet) when the aluminum sheets fell on the employee. The employee was pushed into a rack and pinned. Both legs were broken just under the knee caps.
