He backed out the threaded pin from the kelly drive clevis that was holding the cable drive end. As he tried to push the pin through, his right middle finger was pinched in the clevis and cable. The finger was amputated at the first joint.

The employee's left-hand fingers were pinched between a set form and the bulkhead panel resulting in a partial amputation of the left ring finger involving the tuft of the distal phalanx.

An employee had driven a wrecker out to a job site to remove a piece of heavy equipment that was stuck in mud and wedged against a building. The employee ran winch cables, adjusted the boom, attached chains, and set up a snatch block. He began pulling the equipment out of the mud. A chain that was pulling on the heavy equipment broke, and the employee was struck in the chest by the snatch block. He sustained five broken ribs and a punctured lung.

He began transferring his primary safety to underneath a communication cable; his secondary safety had not clipped in and he fell to the ground about 15 feet below. He suffered a broken left wrist, a skull fracture at the base of the neck, and an injury to the left shoulder.

A tractor trailer drove by on the road below and made contact with the power lines creating an electrical arc that struck the employee. The employee sustained electrical shock and burns to the right arm and lower right region of their back. The employee was hospitalized.

The sheaves of the crane were frozen due to ice and snow conditions, which caused a backlash with the auxiliary cable of the crane. The operator attempted to straighten out the backlash on the superstructure near the drum. The frozen sheave freed itself, allowing the cable to run, and the operator's right arm got tangled in the cable. The operator sustained an open fracture of the humerus and a dislocation of the radius.

The injured employee was feeling the cable while it was moving through his gloved right hand checking for burrs or distortion in the cable when his fingers were caught between the crane cable and the pully of the block. Two fingertips were amputated.

Crew members where unbolting a 40-foot riser pipe in order to separate the pipe in two pieces. When the bolts where removed, the pipes fell apart. The cable used to secure the bottom pipe caught an employee's left hand and pinched it between the pipe and the cable. causing a laceration to the middle finger and injury to the index finger. The employee was hospitalized.
