The mixer was powered on to push the powder out through the port. There was powder adhered to the port, and he tried to get it out using his left hand. The mixer blade contacted his fingers, resulting in deglovings and fractures to the middle and index fingers. The middle finger was medically amputated.

At about 10:10 a.m. on November 4, 2021, an employee was positioning a jackhammer to load into a skid steer bucket. He was bent over at the waist, sliding the jackhammer to get it closer to the bucket, when the sole of his work boot was caught between the jackhammer and the lip of the bucket. He fell forward and his left hip struck the concrete floor, resulting in a fractured hip.

The employee was pinned between the side of the bucket and his truck, resulting in amputations of three fingers down to the first joint as well as a broken right femur.

The upper boom and bucket fell about 7.9 feet, coming to rest on the top of the cab protection rack. The employee suffered a head laceration and was hospitalized.

The injured employee sustained fractured vertebrae, a concussion, and lacerations to the face and head.

An employee was working in a basement, keeping the area secure while other employees drilled down from the floor above. The concrete core drilled out from the floor above fell down, struck the rim of the core-catching bucket, then fell about 15 feet further, striking the employee's hardhat and then the back of his head and neck. He suffered a laceration to the back of the head and a ruptured cervical disk.

The bin needed to be reoriented so the parts door would open; a powered industrial truck was brought over to pull out the container and turn it 45 degrees. When the container moved, a finger on the employee's right hand was caught between the bin and the steel rack. The fingertip was medically amputated.

An employee was prepping a 40-foot solid wood pole to be set with other team members. While the pole was being set into the ground with a track bucket, the employee started to push the pole into place. The operator boomed up to straighten the pole. The rigging slid off the pole and landed on the outrigger of the track bucket. The pole bounced and struck the employee causing the employee to fall to the ground. The employee sustained a contusion to the head and a chipped tooth.

After removing the auger from the kelley of a drilling rig, an employee was setting the kelley onto the cleanout bucket. The employee was trying to insert the pin to attach the cleanout bucket to the kelley and cleaning the pin hole with his left index finger on his left hand. The kelley was lifted while his finger was still in the pin hole, causing the amputation of the finger at the first knuckle.

The front-end loader began to make another pass of the sidewalk, causing the loader bucket to swing and strike the employee. The employee was hospitalized with a concussion and a strained neck.
