An employee was on scaffolding and was using a broom to push an air duct out of the way for a clear view of a pallet on pallet racking. While pushing the air duct, the broom gave way and bent in half, causing the employee to lose balance and fall forward. The employee fell from the scaffold, roughly 5 feet, to the floor and sustained 8 broken ribs, a broken scapula, and a laceration that required staples. The employee was hospitalized.

An employee was preparing to repaint the indoor pool. The pool had been drained and a scaffold was erected inside of the pool. The employee leaned over the railing to drop a broom down to another employee working below when the midrail on the scaffold failed causing the employee to fall 12 feet before striking cross bracing and then fall another three feet to the bottom of the pool. The employee was hospitalized for a broken wrist and internal abdominal injuries.

An employee unloaded a broom tractor from a trailer. He then got off the tractor to check the hub locks on the wheels. When he moved the hub locks, the boom tractor grabbed his leg and took off with him underneath. The tractor then made a 360 circle and ran him over a second time, breaking his pelvis in two places. He was hospitalized and required surgery. The boom tractor was in operation at the time of the incident.

An employee was inside a "dry room" at the press cell area. The room is environmentally controlled and a press is used to make heat pellets from powder. The powder is a flammable solid. The employee was cleaning the powder in the cell area with a broom and scoop and disposing of the powder at the satellite accumulation inside a fire cabinet. It is suspected that static electricity ignited the powder. The resulting fire burned both of the employee's hands.
