While driving back toward the loading dock, the forklift's brakes were activated and the tires slid on the water. The forklift spun and struck two co-workers, knocking them approximately 4 feet off the dock. The forklift fell off the dock and landed on the injured employee's right thigh, resulting in a fractured leg.

The ATV then rolled over the agent. The agent was hospitalized for a fractured scapula.

The brakes were not set at the time, so the tractor rolled slowly back and crushed his body between the tractor and a flatbed that was waiting outside the bay. He was hospitalized for a fractured hip.

Two employees were preparing to install a new die in a press. One employee was pulling the spring block off the top of the die to accommodate the shut height of the brake press. When the employee got the bolts out of the block, the die hung up on the guide pins; as the employee worked the block loose, the die freed and came down on the stop block. The other employee's right middle finger was between the stop block and the top of the die, and the fingertip was amputated.

To cage the spring and remove hazardous energy from the internals of the part, he supplied air pressure to compress the spring and then placed the caging bolt in the hole on the end to retain the spring. When his finger was in the pinch point placing the nut on the caging bolt, the chamber lost air and actuated the spring with the bolt in it. His finger was then caug
