The injured employee was directing the excavator operator in the lifting of the boring rig from the boring pit. During the lift, the chains needed to be adjusted. Two employees were manually tapping holes after destructive removal of studs. They were using a pneumatic ratchet, placed on the socket over the tap, with a stop bar in one of the existing studs. When the injured employee activated the pneumatic wrench, it pulled the employee toward a neighboring stud. The employee's left ring finger was pinched between the tool body and the stud, causing an amputation at the fingertip.
