On June 10, 2021, an employee was stacking a pallet full of textbooks onto a rack about 12 feet high using a reach truck. The employee got out of the reach truck to check on the pallet of books when the books fell toward him. As he ran to get away, he fell on the floor and a book hit his right leg, resulting in a fracture to his right ankle that required hospitalization.
