As the block was coming out, it contacted the bottom of the hammer and caused the timber to rotate. The employee's hand was crushed between the timber and the forklift tine.

About 4-5 millimeters of the finger were amputated.

The employee fell through the fencing, and the employee's fall protection rope snapped/tore. The employee landed on the concrete floor about 14 feet below, suffering a broken rib and a fractured hip.

On April 18, 2021, a maintenance employee was pushing a rail track switch lever to move the track so that an amusement park train could continue on its way. An employee had driven a wrecker out to a job site to remove a piece of heavy equipment that was stuck in mud and wedged against a building. The employee ran winch cables, adjusted the boom, attached chains, and set up a snatch block. He began pulling the equipment out of the mud. A chain that was pulling on the heavy equipment broke, and the employee was struck in the chest by the snatch block. He sustained five broken ribs and a punctured lung.

The hook became unstuck and his right foot was wedged between it and the load block. His little toe was broken.

Two employees were removing a sheave cover on a 310-ton crane in order to reeve the hook block. The hook block was located on stands 7-8 feet above the ground. One employee was removing a sheave cover from above while the other was removing bolts from below. The employee below was struck by the sheave cover and sustained fractured vertebrae, a fractured right ankle, an injury to the ACL and patella on the left knee, as well as a dislocated hip.

An employee was reassembling the screen support roller onto a pillow block. He connected the left side to the pillow block and then went to ensure that the right side of the pillow block was still connected. At about 5:20 p.m. on September 26, 2020, an employee was on a table about 2 1/2 feet high, checking for a blockage in a metal chute that deposited sirloin onto an overhead conveyor. The employee fell, and their right little finger became lodged between the chute and a metal shield used to prevent product from falling off the conveyor. The fingertip was amputated at the last joint.

On September 26, 2020, the cable line of a workover rig failed, causing the block to fall and strike a platform where an employee stood. This caused the employee to fall from the platform. The employee landed about 34 feet below and suffered broken bones and a laceration.

On September 21, 2020, an employee was performing boiler maintenance and was using a 6" hand held angle grinder with a wafer cutting wheel to cut block welds in order to remove the cyclone tubes for replacement. While cutting, the cutting wheel became pinched and the grinder kicked back. The employee lost control of the grinder and sustained a laceration on his right leg above the knee that required sutures and hospitalization.

An employee was working at a treatment facility construction site. A flatbed had transported a single platform about 1 mile to the site. The platform was strapped onto the flatbed and had wood blocks to keep it from moving. During the transport, one of the pieces of wood vibrated out of position. When the employee released the ratcheted straps, the tubular steel frame platform toppled over and off the flatbed. The platform struck and broke his left leg. He was hospitalized and had surgery.
