An employee set up an aerial lift with two outriggers positioned on a road and the other two on an embankment. The employee raised the aerial lift approximately 45-50 feet high to prepare to trim a tree when the embankment gave way and caused one of the lift's legs to slide. The lift fell over and the employee landed on the ground outside of the basket, resulting in compression back fractures at the L1 and L4 vertebrae and damage to the L1, L3, and L4 vertebrae that required hospitalization. As the employee adjusted the come-along, the load shifted and the employee's wrist was caught between the basket and the box, resulting in a broken ulna and torn tendon.

On September 15, 2021, at approximately 8:15 a.m. an employee was driving an aerial lift when the bottom of the front basket caught the edge of a B-Box water flow control valve. The employee continued to raise the aerial lift until the B-Box valve broke and the employee was ejected out of the aerial lift basket onto the front edge of the home's concrete slab. The employee fell 6 feet and was hospitalized for a severe neck injury.

He suffered a broken wrist, broken ribs, a ruptured spleen, and a punctured lung and was hospitalized.
