Stored energy pinched and amputated the injured employee's right middle finger between the gate and the pin.

On January 10, 2019, an employee was helping in the operation of a sheet metal slitting machine and was operating a sheet metal re-coiling machine. He was trying to tighten up the sheet metal coil before banding it. As the coil turned, his right pinky finger was caught in a pinch point between the coil and the tensioning bar of the re-coiler. The finger was cut about halfway through and was later surgically amputated just below the first knuckle.

When the gate closed, the machine started moving a bundle of pipe down the rollerway where the injured employee stood, pushing him off his feet and pinning his left leg against the roller. This caused a laceration to the side and back of the left leg above the knee and a fracture to the right ankle requiring surgery and hospitalization.

At that time, the employee's glove was caught between the lapse of the coil and was crushed by the separator, causing an amputation of the left ring finger.

As the metal banding was cut, it retracted to the floor and bounced up, cutting the employee at the knuckle on the right middle finger. Company-provided leather gloves were not worn at the time of the incident. The employee was later hospitalized for a possible infection and received antibiotics.

An employee was trying to install a new roll of banding metal to the banding roll. A crane operator was lifting the roll on to the machine. When the employee tried to remove the nylon lifting sling, it became stuck. The crane operator tried to remove the stuck sling with the crane. When the operator pulled up on the sling, it pulled the roll of banding off the spool. The banding fell approximately 16 inches onto the employee's foot.

On or about April 15, 2015, at 3:30 p.m., an employee climbed up on a 4-foot railing to cut the banding off of a bundle of lumber when he slipped and fell on his back. He sustained five broken ribs and five cracked vertebrae.

An employee had just finished unloading a shipment of cardboard boxes. The cardboard boxes were on pallets; the individual pallets and stacks of cardboard boxes were secured with banding straps. The employee grabbed and pulled on a banding strap on one of the pallets to try and straighten the pallet by a few inches to align with the other pallets.
