An employee was removing the rear axel on a forklift for maintenance purposes. While the forklift was blocked underneath, the employee utilized a chain-link hoist to lift the rear of the forklift. In the process of doing this, the employee's finger was caught between the chain-link hoist and the forklift counterweight, resulting in a left ring finger amputation down to the first knuckle.

An employee was greasing a trailer at a loading dock. A forklift entered the trailer causing the trailer to bounce and the height locking mechanism to disengage. The trailer frame then came down on the axel and the employee's left middle finger was caught and crushed between the axel and frame. The injury led to an amputation.

On February 11, 2019, at 1:30 p.m., a journeyman mechanic was performing a diagnostic test on a tractor (truck). While standing inside the engine bay of the truck between the frame and the left steer tire axel, the vehicle started and the truck moved forward pinning the employee under the steer tire. The employee's upper leg was crushed and the hip and ankle were fractured.

An employee was under a tractor-trailer placing masking tape on items to be painted while a second employee was working on the airline for the air brakes. The second employee then tested the airlines, which caused a rear axel to fall on the first employee's left leg. The employee suffered lacerations and a possible fracture.

An employee was removing the outside dual wheel on a farm tractor to get access to the inner dual tire. The employee had the rear wheel of the tractor raised up with a jack and was using a truck-mounted boom to help remove the outside wheel/tire assembly. After the employee unbolted the wheel and maneuvered it off the axel, the wheel tilted and struck the employee. The employee fractured five ribs, punctured a lung, and suffered a laceration behind the right ear.

An employee was helping a second employee remove bolts from a steel plate attached to an axel fixture. One employee was using an air wrench to loosen bolts while the second employee was using a wrench to hold onto the bolt. The first employee loosened the bolt, and the plate slid down and struck the second employee's right index finger, amputating the tip.
