An employee was operating a road grader. The grader stopped and the employee dismounted to check it. The grader then began moving. The employee chased it down and jumped up to stop it, but fell under it. The employee suffered a broken right leg.

On October 1, 2020, an employee was using a radial arm saw to cut 2x4's. An employee was removing a piece of cut wood when the teeth of the saw grabbed his gloves and pulled his hand into the saw. The left middle fingertip was amputated immediately and the ring finger was surgically amputated.

An employee was using a radial arm saw to cut 2 X 4's and 2 X 6's, both 4 feet and 8 feet, into 2-foot pieces when the blade grabbed his glove, pulling his right hand into the saw. This resulted in a middle fingertip amputation midway down the nail. His ring finger on his right hand was cut almost completely off at the first knuckle.

An employee was using an air hose to clean up/blow off sawdust from the work area. When he sat the hose down to go turn off the shut-off valve it started jumping around and hit his buttocks, causing a 1" to 2" laceration that required hospitalization.

An employee had just finished nailing a fence panel using a nail gun. As he extended his arm down into a resting/hanging position with his finger still on the trigger, another employee walked behind him and was struck in the lower leg with the nail gun, which actuated and drove a nail into his leg. The injured employee was hospitalized overnight.

In a unit office/housing unit where inmates are housed, an employee was seated at a desk when an inmate walked into the office. The inmate punched him first in the face and then repeatedly punched his head. His nose was injured and he lost consciousness. He was hospitalized with a concussion.
