An employee was filling a molten unit. The discharge valve was closed at the time, and the line over-pressurized. The line burst, spraying the employee with scalding water that caused second and third degree burns to the ears, neck, hands, and chest.

Two employees were in a trench attempting to cap off an underground water line. Part of the side of the trench came loose and pushed one employee against the side of the trench. The other employee was stuck in the mud up to his waist. One of the employees was hospitalized and had surgery for broken ribs and a lacerated small intestine.

On 10/30/15 at approximately 1420, an employee was removing an equipment guard that was 6 inches off the ground. He removed the top bolt while supporting the bottom of the guard with his right hand. When the top bolt came loose and dropped on his right hand it pinched the top of his ring finger resulting in a partial amputation.
