A piece of a component the employee was holding came loose, causing the employee to lose his balance and fall backwards to the ground 55 inches below. An employee was striping lines in a soccer field. He placed a metal stake in the ground, using string as chalk. The string did not reach as far as needed and when the employee tugged on the string, the stake came out of the ground like a slingshot, striking his left bicep. The stake traveled through his arm and punctured his lung.
