An employee was standing in a basket attached to a forklift. The lift lowered the basket to the ground and the operator disconnected the basket straps. The employee in the basket was attached to the forklift not the basket. As the lift drove away, the employee was pulled and fell out of the basket hitting his head and shoulder.

An employee had removed a cover for the drive belt and the exit roller so the timing could be adjusted for the inserter. His left index finger was caught between the drive belt and the rotating exit roller, and his index fingertip and approximately two-thirds of the bone were amputated. The machine was not locked out at the time.
