On 9/08/2021, a concrete truck driver was repairing a rear light approximately 7-8 feet above the ground. He lost his grip and his hand got caught between the roller and the drum. He lost a fingernail and required amputation to the right ring finger.

Three employees had just retrieved a wooden beam from under a section of ductwork. One of them was bending over to pick it up when the duct section shifted and a piece of angle iron slid off it onto that employee's left thumb.
