An employee was walking by a railcar commodity offloading area. A 2-inch rubber hose funneling phenol into a 55-gallon drum became dislodged from the drum and sprayed liquid phenol onto the employee. He was hospitalized, having suffered second-degree chemical burns to the back of his neck and first-degree chemical burns to his face and arms.

An employee opened the rear door of an open top refuse container and was struck by a roll of plastic sheeting that weighed approximately 300 pounds. As the roll struck him, the employee fell backwards and struck the back of his head on a concrete pad. Another roll of plastic sheeting dislodged from the container and landed on top of his legs. The employee sustained fractures to his right ankle and pelvis, as well as a laceration to back of his head.

An employee was attempting to clear a pellet clog in a conveyor using a wrench when the hand wrench contacted the moving conveyor chain, causing the wrench to move forward and resulting in the employee's right index finger being pinched between the wrench and the conveyor housing. The employee sustained an amputation of the distal right index fingertip.
