| On May 13, 2019, an employee was asked to locate a motor in the storage room. About 20-30 minutes later a maintenance employee came into the storage room and found the injured employee leaning against a rack and bleeding. A ladder was found on the floor and the injured employee later said he had been on the ladder about 4 feet high. The employee was hospitalized with a skull fracture. | |||||

| On September 29, 2017, at 4:30 PM, an employee was kneeling and setting up a hydraulic compacting press with his left hand on the control panel. As he moved to the standing position, his left hand flipped the core rod switch into the down mode. The core rods then moved down and partially amputated his right little finger. | |||||
