An employee was on a 5-foot ladder repairing a hydraulic valve and changing the internal packing. He used pressurized air to push the piston up. He then bled the fluid out of the valve. Once he removed the last bolt, pressure released, and the valve housing struck the employee in the rib area knocking him off the ladder. The employee sustained bruised ribs and back pain. | |||||

An employee was using an oxy/fuel gas cutting torch to dismantle a railcar when a small pinhole developed in the oxygen line near where the hoses meet the torch, causing a leak. Oxygen then migrated up the employee's sleeve inside his protective leather-cutting jacket and ignited. He received second and third degree burns on his right shoulder, armpit, and upper chest. | |||||

An employee was using a cutting torch to disassemble a steel processing tank. The employee was working on the top of the tank, approximately 7 feet above a lower level. The employee walked over a section of the tank that had previously been cut but was still attached to the structure. The section gave way and the employee fell approximately 7 feet to the ground, fracturing the pelvis and left elbow. Fall protection was not worn at the time of the incident. | |||||
