The employee was securing the broken cam-lock mechanism when a plug shot out of the pipe and broke the employee's finger. The employee was hospitalized for surgery.

While working in the Rigger Shop, an employee was using channel lock pliers to disconnect two pieces of pipe that were bolted together. The pipe was raised by a crane. The employee's finger was pinched between lower handle of pliers and the portion of pipe supported by the crane. The employee suffered an amputation to his left ring finger.

On 5/22/2017, at approximately 10:30, an employee was installing an auger inside the housing of a larry car. The employee rigged the auger with chain falls and come-a-longs to position it inside the housing. During the installation, the employee's left index and middle fingers were caught and partially amputated between the auger and the housing. The equipment was locked out and not powered at the time of the incident.

An employee sustained injuries to his ring and index finger while cutting a piece of wood on a table saw. At the time of the injury on 1/14/15 it was believed that the employee had sustained lacerations to the fingers. It was later found out on 2/4/15 that the injury to the index finger resulted in an amputation while the employee was under medical treatment.
