At approximately 12:55 a.m., an employee was helping another employee load steel beams onto a tractor trailer. He picked up a beam with an overhead crane using the crane's remote control box. After attempting to maneuver the beam into an open space on the trailer, he determined the beam would not fit because it was too wide. He placed the beam on top of a smaller pile of beams. During this process, the chain holding the load caught the flange of an adjacent beam, tipping both beams and a tube.
