On April 5, 2016, at around 11:00 AM, an employee and a coworker were loosening jam nuts while standing on a tractor hitch. When the employee loosened a jam nut, he lost his balance and fell backwards approximately 3 to 4 feet, landing on his back. He suffered a collapsed right lung and 2 or 3 fractured ribs.

Employees were going to remove an aeration pipe (18 inch in diameter and 10 feet long) that had potatoes on it with a 3/8" chain & a skid steer with forks. The chain was secured around the pipe and around both forks on the skid steer. After the pipe was free of the potatoes, the skid steer driver began to back up, while another employee guided the pipe.
