While traveling, the first employee made an abrupt turn in front of the second employee. The second employee was unable to stop or swerve to avoid collision and ran into the rear of the first employee's forklift, causing it to roll over. An employee was adjusting the lumber on a chain conveyor when the chain came off the sprocket. His left hand was caught by the chain and sprocket pulley as he tried to put the chain back on the sprocket, partially amputating his pinky finger (first knuckle), fracturing his ring finger, and damaging the ligaments on his other fingers. The chain conveyor was moving at the time of the incident.

An employee was using the "button machine" to crimp pieces of fabric around buttons. One of the pieces of fabric was off center and the employee reached under the guard to slide the fabric back into place. The employee pressed the foot pedal that operates the machine while her hand was still in the machine and amputated the right index fingertip and fractured the right middle finger.
