An employee was using the "button machine" to crimp pieces of fabric around buttons. One of the pieces of fabric was off center and the employee reached under the guard to slide the fabric back into place. The employee pressed the foot pedal that operates the machine while her hand was still in the machine and amputated the right index fingertip and fractured the right middle finger. |
