On June 5, 2018, at approximately 2:00 p.m., an employee slipped on soybeans while exiting a flat storage building and his left leg went into the running floor auger resulting in amputation of the left leg. The auger was unguarded at the time of the incident.

An employee was doing preventative maintenance on a conveyor. After applying oil to the chain and sprockets, the employee used a rag to clean up spilled oil. The rag and the employee's right middle finger got caught in the chain and sprocket. About 1/4 inch of the fingertip was amputated. The machine was not guarded at the time.
