An employee was guiding a steel stair suspended from a crane hook into place to be connected with bolts. He used his sleever bar to guide and make sure the stair riser cleared the opening in the steel, but the top stair riser got hung up on the support beam. He pushed the stair riser with his foot. A maintenance employee was removing a conveyor belt. The employee performed lockout/tagout and removed the guard from the V belts and pulley on the side of the head pulley. The employee then began to manually move the V belts over the pulley. His left pinky and ring finger were caught between the V belt and pulley resulting in amputation of the top 1/4 of the employee's left pinky finger.

An employee was operating an electro-pneumatic, air-powered, mold-controlled system. As the mold was coming out of the machine (the mold is heated up to 200 degrees), it became lodged between the doors. When the employee attempted to dislodge the mold, the doors slammed shut on his left hand, lacerating his hand and causing first degree burns on his left wrist and forearm. He was hospitalized.

An employee was standing near the area where an auger crew was preparing to drill a hole to install a light pole. A pin holding the auger broke, causing it to fall. When the auger fell, it hit a security fencing and then landed on the employee's right foot, smashing the fourth and fifth toes.
