On March 24, at around 7 a.m., an employee was working on an HVAC piping system that was under a pressure test. Believing that the pressure test was completed, he removed the end cap from the pipe. There was still pressure in the pipe, which caused the end cap to fly off and strike him in the mouth area. He suffered lacerations to his mouth and cheekbone.
