An employee was in a manlift basket with a co-worker, washing a drilling rig in preparation for the rig to be moved off site. The manlift basket became caught on the rig, and when the basket released, it bounced and caused the employee to be ejected. Employees were using a rough terrain forklift truck to pivot a fork of another rough terrain forklift truck. The fork was tied to its carriage. When the fork was untied it came down and struck an employee standing near the truck and broke his leg near his ankle. |
