As the load was about four feet from the ground, a rigging failure caused the steel to buckle and strike the employee. The employee was launched into the air about 15-20 feet. The employee sustained a broken arm, collar bone and pelvis.

On July 12, 2017, an employee was using a wrench to remove a line plug from a hydraulic cylinder while it was resting in a wooden cradle (block) on top of a machine's sub-frame. The wrench breaking the line plug loose created a jarring motion that caused the 620-pound hydraulic cylinder to shift and fall (3.5 feet) off the elevated surface and onto the employee's lower leg, fracturing the right tibia. The employee was hospitalized.

Approximately 1/4" of the employee's left middle fingertip was amputated. The basket is made of steel and is shaped like a small garbage can measuring 2' x 1.5' and weighing 200 pounds.
