On July 12, 2017, an employee was using a wrench to remove a line plug from a hydraulic cylinder while it was resting in a wooden cradle (block) on top of a machine's sub-frame. The wrench breaking the line plug loose created a jarring motion that caused the 620-pound hydraulic cylinder to shift and fall (3.5 feet) off the elevated surface and onto the employee's lower leg, fracturing the right tibia.
