At approximately 9:45 a.m., on 2/21/2018, an employee was performing maintenance on a press. The machine was properly locked/tagged out and he was wearing all assigned PPE. The employee was using a pry bar to help remove a component of the press. As he was pressing on the pry bar, it slipped, and his left-hand pinky finger was then smashed between the pry bar and the edge of the mold he was working on, resulting in a partial pinky finger amputation. | |||||

An employee was operating a machine that uses air pressure to push small plastic cleaning wads through half-inch diameter pipes to remove debris and oil. A metal rod from the machine broke off and remained in the pipe. When the cleaning plug and air pressure were applied to the pipe during the cleaning process, the pressure built up and eventually shot the metal rod out of the pipe.
