Employee 1 was using a front-end loader inside a concrete storage bin to scoop up a boulder of burnt soybeans that had clumped together. Employee 1 could not scoop the boulder into the front-end loader's bucket. The confined space attendant and a supervisor went into the storage bin to assist the front-end loader in scooping the boulder into its bucket. Another soybean boulder, weighing approximately 100 pounds, fell down and hit the attendant and the supervisor and they both sustained fractures

An employee was trying to free a screw that was clogged with rice hulls. The employee shut off the power to the screw and went to pull on the belt that was connected to the pulley that ran the screw. While the employee was pulling on the belt, the screw became free. The stored energy from the bound screw caused the belt and pulley to rotate while the employee was holding the belt. The employee's left index finger entered the pinch point of the belt and pulley, amputating the left fingertip above

An employee climbed up onto the carriage and used his weight to force the carriage blade to move. When it did, the employee stepped off the carriage and his left big and second toes were amputated by the blade.

An employee was using a forklift to empty an aluminum scrap hopper. He exited the forklift and walked under the elevated hopper. His left hand was on the fork as he struck the lever that dumps the hopper bin. When the hopper dumped its load, it created a pinch point between the hopper and the forklift forks. The employee's left fourth digit was caught between the hopper and the forklift and had to be surgically amputated.
