An employee was cleaning parts for the Piab system (pneumatic transfer system for raw materials) using sani-wipes as part of the allergen clean process on the bar line. On 3/10/16, at 6:15 a.m., an employee went up a ladder to unlock a hatch and access the roof for work. He unlocked and opened the hatch, saw that the ladder was not extended 3 feet above, and went back down to readjust it. He lost his footing and fell 8 to 10 feet, breaking his left leg and right heel. He was hospitalized.
