Employee 1 was replacing a bucket (480-volt breaker) while Employee 2 was observing from outside the arc flash boundary (36"). Employee 1 completed the work and was closing the door when an arc flash occurred. Employee 2 sustained first degree burns on the forehead and cheek and first and second degree burns to the hand. Employee 2 was hospitalized. Employee 1 was not injured.

An employee was feeding paper through a winder. The paper did not transfer from the back of the drum to the roll; the employee was trying to transfer it when the front drum came into contact with her left arm and pulled it into the nip point between the roll and the drum. The arm was bruised.

An employee was partially pinned underneath the front three sheets of bundled steel plates that slid off of the front of a flatbed trailer while the crane was attempting to set them down on the trailer. The employee suffered a fractured cheek bone and ocular socket, a large laceration on his back, and a dislocated shoulder.

An employee was dismantling scaffold when a large exhaust fan turned on. The fan blades amputated the employee's right middle fingertip below the nail bed, fractured the right ring finger and lacerated to the right index finger.

An employee was performing maintenance on a grinder machine, which was lifted up with a pin placed underneath. The machine fell and pinched the employee's fingers, amputating the right-hand middle and ring fingers at the distal joint.
