Employees were moving a pipeline inspection tool barrel. The injured employee was pulling on the rigging in order to allow a forklift to move the barrel through an open door. As the barrel was coming through the door, the rigging slipped off the fork and the barrel landed on the employee's foot. The employee's two left toes adjacent to his big toe were amputated.
