A boom attachment on the top shelf of the racking fell down as the two employees were pulling on the crossbar to lower one of the shelves in the middle of the rack. The boom fell and hit the crossbar, jamming it in an employee's left collar bone and chest. The cross bar then fell and landed on the other employee's feet, resulting in a fracture and hospitalization.
